Abstract

Absolute glaucoma is the final stage of chronic optic neuropathy, characterized by irreversible damage to the optic nerve head, total vision loss (no light perception/NLP), and often accompanied by persistent eye pain due to high intraocular pressure (IOP). This case report describes a 76-year-old female patient who presented with complaints of severe headache on the left side and swelling and redness of the left eye, with a 7-year history of progressive glaucoma. Ophthalmological examination of the left eye (OS) revealed zero visual acuity (NLP), accompanied by eyelid edema, bulbar conjunctival edema, a corneal defect, a collapsed anterior chamber, an unresponsive mydriatic pupil, and an IOP of 45 mmHg. The patient was diagnosed with absolute glaucoma in the left eye and received palliative medical treatment consisting of timolol maleate eye drops in the left eye and artificial tears in both eyes. This case highlights that chronic, uncontrolled increases in intraocular pressure can lead to permanent blindness that is often resistant to standard therapy. Furthermore, it underscores the importance of early detection, routine examinations, and patient education regarding treatment adherence as key strategies to reduce the incidence of glaucoma-related blindness in Indonesia.
