This study aims to analyze the relationship between the implementation of healthy lifestyle policies and employees' physical fitness levels, while identifying key lifestyle determinants at a state health polytechnic in Indonesia. Using a quantitative, cross-sectional design supplemented with a retrospective analysis of routine health records, 238 employees were selected via stratified random sampling from a population of 440. Data were collected through routine health parameters and validated instruments (IPAQ Short Form, Healthy Eating Index, PSQI, and DASS-21). Repeated Measures ANOVA was conducted to evaluate quarterly health record parameters, while Multiple Linear Regression was performed to determine the contribution of lifestyle factors to physical fitness. Results revealed that most employees maintained a "fair" lifestyle (42.9%), with gender as the sole significant predictor of lifestyle compliance (β = 0.131, p = .032), where female employees showed higher compliance scores. Retrospective analysis indicated that policy support maintained fair cardiovascular fitness levels (55.7% in Quarter IV). Conversely, periodic health screenings did not significantly reduce overall metabolic risks: body weight increased over time (p < .001), resulting in an overweight average BMI (26.0 kg/m2), alongside significant quarterly fluctuations in random blood sugar (p = .002), systolic blood pressure (p < .001), and total cholesterol (p = .001), all remaining above normal thresholds. In conclusion, although institutional health policies and periodic screenings maintain fair cardiovascular fitness and short-term clinical stability, the institution must transition from passive monitoring to structured, gender-sensitive active interventions to sustainably manage metabolic risks.References
