Abstract

Waiting time for treatment is a service quality indicator that remains problematic in the radiotherapy unit , where the standard is set at ≤7days. This study aimed to analyze radiotherapy waiting time using a Lean Healthcare approach. An explanatory sequential mixed-methodsdesign was applied, beginning with a quantitative descriptive study of 107 patients determined using the Lemeshow formula, followed by aqualitative study through in-depth interviews, focus group discussions, observation, and document review involving 8 key informants and 6patients. Data were analyzed using Value Stream Mapping, Value Assessment, and a fishbone diagram. The results showed that 62.6% of patients experienced waiting times exceeding the standard (median 9 days; mean 9.8 days; SD 5.8 days). The main bottleneck wasidentified at the delineation stage of the Treatment Planning System, caused by the absence of full-time attending physicians and compounded by reliance on a single Linear Accelerator unit. This study recommends integrated Lean Healthcare-based improvements in human resources, policy, facilities, and information systems.
