A Literature Review: The Impact of Pharmacist Homecare on Knowledge and Medication Adherence in Elderly Hypertensive Patients

Abstract

Medication adherence among older adults with hypertension remains suboptimal despite the availability of effective antihypertensive therapies, highlighting the need for patient-centered pharmaceutical care. This review aimed to synthesize evidence on the effectiveness of pharmacist-led homecare services in improving medication knowledge, medication adherence, and clinical outcomes among older adults with hypertension. A structured literature review was conducted using PubMed, Scopus, and Google Scholar to identify relevant studies published between 2014 and 2024. Eligible studies evaluated pharmacist-led homecare interventions and reported outcomes related to medication knowledge, medication adherence, or blood pressure control. Ten studies met the inclusion criteria and were narratively synthesized due to heterogeneity in study designs and outcome measures. The findings consistently indicated that pharmacist-led homecare improved medication knowledge and medication adherence. Effective interventions generally incorporated individualized patient education, comprehensive medication reviews, adherence monitoring, home visits, follow-up communication, and caregiver involvement. Several studies also reported improvements in blood pressure control. Overall, pharmacist-led homecare shows potential as a pharmaceutical care model for optimizing hypertension management among older adults through integrated educational, behavioral, and monitoring strategies.
